In order to complete the new Z83 form, both Part A and B must be filled out in full. Additionally, two questions relating to the condition that prevent re-appointment under Part F must be answered in full. Page 1 of the form must be initialed, and failure to comply with any of the aforementioned requirements will result in disqualification from the application process. Part C, which pertains to method of correspondence and contact details, must also be filled out in full. Please note that, as of 01 January 2021, DPSA has approved the new Z83 Application Form (which can be obtained from any Public Service department).

The communication from HR of the department regarding the requirements for certified documents will be limited to shortlisted candidates.
